What forms to provide for California maternity leave, PDL, and CFRA?

Processing maternity leave involves coordinating overlapping timelines for the period before birth, recovery, and baby bonding. Determining exactly when to hand out initial WH-381/382 forms for FMLA and PDL versus separate eligibility forms for CFRA creates confusion. This complicated paperwork process leaves employees stressed about how it all works and what to expect financially.

What people tried

Every workaround mentioned in the threads below. We haven’t tested any of them — and nobody here is claiming they worked.

  1. 1
Using internal LOA request forms and medical certification forms instead of or alongside state/federal forms
  2. 2
Creating personal charts to track how pregnancy disability leave runs concurrently with FMLA but not CFRA
  3. 3
Scheduling direct meetings with staff to go over all documentation and sending follow-up emails with attached forms
  4. 4
going to a parental leave consultant
  5. 5
asking district HR to calculate the dates and pay
